Q: What are Bioidentical Hormones?

A:    Bioidentical hormone preparations are medications that contain hormones that are an exact chemical match to those made naturally by the human body. Synthetic hormones are generally made by drug companies, are approved by the FDA, and are sold in standard doses.  Most bioidentical hormone preparations are made at special pharmacies called compounding pharmacies, which make the preparations on a case-by-case basis for each patient.  The key to understand is that bioidentical hormones are identical to the molecular structure of hormones made by the human body.  Often called “Natural Hormone Therapy” because bioidentical hormones act in the body just like the hormones we produce naturally.  

Q: Why Aren't Compounded Bioidentical Hormones FDA approved?

A:    It’s not that compounded bioidentical hormones are not FDA approved. A person’s individualized dose has not been FDA approved.  Bioidentical hormone substances have been FDA approved for use in the USA for decades. It is up to a practitioner to prescribe the proper individualized dose which many times is referred to as “off-label” use.  The FDA cannot approve anything that does not come in a standardized dose.  That doesn't mean that compounding is bad. Compounding can be useful for patients who are allergic to an additive in an FDA-approved product, but the purpose of compounding is to do it on a patient-by-patient basis, so there's nothing that's submitted to FDA to evaluate, so they're not technically FDA approved.

Q: What happens in the body regarding weight gain/loss when estrogen and progesterone are out of balance?

A:    Low Progesterone/High Estrogen causes weight gain.  There are a variety of reasons for this occurrence: Sugar cravings due to increased insulin levels (Progesterone helps to lower insulin), more fatty tissue growth causing fat cells to produce more Estrogen, fluid retention (Progesterone is a natural diuretic), decreased Thyroid function (Low Progesterone causes the liver to produce Thyroid Binding Globulin), and Poor sleep quality.

Estrogen is the primary female sex hormone. It gives women hips and breasts, buffers mood, and keeps you on task by regulating serotonin. Estrogen is responsible for the first half of your menstrual cycle, it boosts sex drive, helps maintain bone density.

Estrogen dominance is when you have too much estrogen compared with progesterone. Having a little bit too much estrogen in the body causes a number of symptoms, and makes losing weight very challenging, if not impossible. 

Progesterone is important for your overall sense of equilibrium or well-being.  It is the hormone that allows you to soothe yourself.  Progesterone raises body temperature making it more “thermogenic” which is a boost to your metabolism, and helps your Thyroid perform more efficiently.  Progesterone is a natural diuretic, which means it helps you release excess fluid in your body.  Progesterone is very beneficial in treating premenstrual symptoms (PMS) such as mood swings and migraine headaches, and It dramatically decreases pre and post-menopausal symptoms.

When progesterone is too low, you’re more likely to develop endometriosis and problems with uterine bleeding, possibly even cancer. When you have too much estrogen, you have a greater risk of infertility and endometrial cancer, a malignancy arising in the lining of your uterus, and breast cancer.  You need an expert to diagnose and treat your specific condition.

Q: Could being tired all the time be caused by an estrogen and progesterone imbalance?

A:   Normally you have 100 to 500 times more progesterone than estrogen in your body. If your ratio is off, you may have symptoms of estrogen dominance, including being tired, weight loss resistance, depression, fatigue, breast tenderness.  Many women report feeling like they have PMS, or that they fly off the handle more easily than they used to when their Progesterone levels are low.  This can also be true for low Estrogen. 

Being tired all of the time may also be related to Testosterone deficiency and Hypo-Thyroidism (Low Thyroid Function) as well as Adrenal Dysfunction in women of all ages.

Q: What other hormones play a role in weight loss/gain?

A:    Other hormones that affect weight loss/gain are Cortisol, Insulin, Leptin, Testosterone, Thyroid, and Vitamin-D3. High leptin causes weight gain and excessive hunger. Leptin is nature’s appetite suppressant. When you’ve had enough to eat, leptin signals your brain to stop eating.

When you are overweight your fat cells produce excess leptin.  When your brain gets bombarded with leptin signals from too many fat cells, it shuts down from overwhelm. Leptin levels keep rising, receptors stop functioning, your body doesn’t get the leptin signal, and you don’t feel full. You keep eating the wrong foods in an addictive pattern, and you keep gaining weight.

Cortisol is the main stress hormone and is released in response to stress, but most of us run around stressed all the time. All those stress hormones wreak havoc over time, and make you store fat—especially in your belly. High cortisol is also linked to depression, food addiction, and sugar cravings, so that you overeat the wrong foods like cookies and processed foods. What’s the net result? You get fat.

Low Cortisol levels also contribute to fatigue and weight gain.  You need an expert to determine the root-cause of why your body is expressing itself with a variety of symptoms.

The body must remove excess glucose from the blood stream to protect from organ damage over time.  A vicious cycle ensues in that Insulin resistance causes weight gain, greater sugar addiction, and further increased Insulin resistance.
